This might not be necessary since you guys are probably well-versed on this awesome trinket but this is my first go with it (thanks to [MENTION=13311]srs bsns[/MENTION]!) and I found some pretty awesome stuff. So I'll do a little guide.

Passive:
It's not as good as Discerning Eye of the Beast - Item - World of Warcraft for damage.

My Frostbolt (out of BG, non crit) with double int trinkets is 330 and with Touch of the Void - Item - World of Warcraft it's 323. But it's fairly close.


On use:
Yea ok. 2008 damage total. Peaked at 500 dps and settled at 88. Not sure why it fell off, my dueled target might have killed a few or maybe he was out of range of a tentacle. But that's just insane.


Tips and Tricks:
For a Mage this should play like Ring of Frost - Spell - World of Warcraft and it's awesome at that. Drop it on your healer to force melee off. Or at the tunnel to stop inc flag returners. Or keep people off a base cap in AB.
It breaks Stealth. Yes. It's awesome. Did not see target until it meleed for 80.
The tentacles go with you. On use starts the spawning, they drop at your location. If you move they will spawn at your current location. Takes about 5 seconds to get them all out. Can maybe drag them up tunnel? Or burn them all at once at a spot for max damage. Works like the old Lifebloom, and how flowers would follow your location on use.
You do have the option to switch back to another trinket after use. Need to be out of combat to do so. Swap it back after the 2 min mark.

biggest thing you mentioned people miss is the fact it moves with you, commonly I see people use it and stand in 1 location and allow people to just step out of it. I noticed on rogue I had dueled a warlock and he started running I popped trinket when I got in melee range I got pet seduced but void killed em after spawning while he was running.

Lol works pretty well on a restoration sham
You put it around you and the rogue has to step through the tendrils
 
If you're about to die, hide in a confined space (like restoration hut) and put up the tentacles. Most people wont enter and if they do, they take a massive amount of damage. It's great for restealths or to bandage yourself.
 
For mage you can poly your target then spawn them around the poly meanwhile. Open with a frost bolt, nova them, you can rack in much more burst that way.

Remember as long as even one tendril is in combat ( from attacking someone or being attacked itself), you will also remain in combat. Lets say you go for efc kill and pop them. Then try to meld and reopen, you won't be able to stealth most likely.

I wouldn't say the damage is massive but the damage is not one to be ignored even for a short time, especially on a mage.
