I've easily farmed 1000 uncommon drops over the past 2 months, in a variety of zones... about 100 were "unscaled" (most often from Hillsbrad rares) with ilvl = rlvl+5. The rest were all ilvl = rlvl+3. Not a single one was ilvl = rlvl (even looking up "scaled" uncommons on WowHead will show ilvl = rlvl+3)...
My entire motivation for even posting was my surprise at seeing low-level uncommons listed on the Auction House with ilvl = rlvl, and trying to identify their origin (which turned-out to be crates, instead of drops).
...and now I'm simply trying to share/confirm that "ilvl = rlvl+3" is a much better metric than "not ilvl X or Y" for recognizing the "genuine drops" of scaled low-level uncommons on the Auction House.
I know that "Nobody's Perfect", but I gotta' confess... I'm a bit disillusioned by your pushbacks here.
EDIT: I may have been ignorant of an additional "property" of some special item drops... stay tuned!

Sin'dor I looted Abomination Cleaver late last year that was scaled ilvl = rlvl-1 (18/19). Since logging in post 8.1 it is now 19/19. Is this an anomaly?

As mentioned earlier in this thread... the drops from Dr. Whitherlimb are (quite annoying) exceptions to the general "rules".

Are those random zone greens or the ones from like quests or rare that can go epic? Those are the ones that seem to follow the ilvl=rlvl rule. Like the helm I have on my mage it is originally ilvl 30 https://www.wowhead.com/item=57462/goldrinn-acolytes-cowl but is ilvl 20 on my 20 mage. The scaling for random zone greens makes no sense to me

I haven't been "chasing epics" (I wear plate), so if that's one of the properties of those types of items, then that's exactly the type of explanation that I was hoping to be given... Thank you!
Do you remember if was actually rlvl-20 before you equipped it? Or could it have been 17//20?

For items that scale directly to your level, they do scale at ilvl = rlvl. If you go to the wowhead page for https://www.wowhead.com/item=3053/humberts-chestpiece&bonus=3642&ilvl=19&lvl=19 (let me see if i can get the tooltip correct...) and adjust the slider, you'll see that at level 19, it has ilvl 19 as well. Those will have blue as rlvl+6 and epic as rlvl+11. Those items are typically dungeon boss drops, quest items, single source drops, and zone specific drops. Foamspittle staff (single source), Fallen Apprentice Robes (quest), Leggings of the Fang (dungeon drop) and Shadowfang (SFK only drop) all scale to the exact level and follow that ilvl scheme (even if some of those can't be green or upgrade).
There are also BoEs that scale to more or less your level - these are all the world drops iirc. The blues scale to rlvl+8, and the required level is typically 12/17/22/27/etc for weapons (iirc), and armor is 15/20/25/30/etc. So Keller's Girdle at rlvl 20 is really good with ilvl 28. Greens have several possible scales and often seem to skip things for some reason. I don't know that anyone has managed to figure out the exact formula's and which items fall under what - but those are the typical items that you see as ilvl = rlvl + 3.

I somehow never experienced an uncommon drop scaling "directly to your level", and presumed that was only a factor once the "upgrading" actually "proc'd".

So I started using rlvl as required level (which wowhead doesn't display) - but now I'm a bit confused. What are you defining rlvl and clvl as?
If rlvl = required lvl and clvl = character lvl, then for cases like Humbert's: ilvl = rlvl = clvl

Actually, you can get the tooltips to (slightly confusingly) sneak-in the clvl after the (useless) rlvl by adding &lvl=## in addition to the usual &ilvl=##...
...which is what started this whole mess. I'm used to thinking that an "ordinary" uncommon drop, scaled for a level 20 character in a level 10-60 zone, might (for example) have an item-level of 19 and a required-level of 16... so clvl//ilvl//rlvl would be 20//19//16.
You can imagine my reaction when you start talking about apparently-similar items being a seemingly "inconceivable" 19//19//19.
Combine that with the fact that my only previous "exposure" to such items were dozens of obvious crate-listings on the Auction House, and I'm surprised that I'm still sane at this point.

Yeah that's Brightplate girdle's ilvl from before scaling quest rewards so was probably obtained pre-patch

https://www.wowhead.com/item=38513/boarhide-leggings shows as 4% drop when in reality its closer to 30%, who knows maybe it got buffed like boarhide

Maybe...but after farming it, the drop rate REALLY sucks. My personal anecdata puts it like 1-3% or something.
