Done, reformatted it a bit too
I did it in less than 10min, so let me know if I missed anything or made some mistake

Also split up Warforge and Epic upgrade since they are different to me
Apexis rares can proc warforge but don't gain any ilvl from it, while an epic proc usually increases the ilvl quite a bit.
