Farming epic items

Hello,

I recently got back to the 20 bracket and noticed people have ilvl 31 epic items on their lvl 20's. This got me wondering what is the best way to farm these items and under what conditions do they even drop?

Every quest green has a chance to upgrade to rare or epic.

Some unique items that only drop off of particular mobs also have a chance to upgrade to rare and epic. These are usually BOE items too.
