Where do you physically get the quest though? The NPC just outside of The Violet Hold hasn't been displaying as having available quests to me. Can you grab from the journal?

No weekly quests for Legion ever again now actually. They got rid of them.

I'm curious if you've completed M Antorus will the quest reward be a mythic cache, or does it stay heroic no matter what?

If it behaves like the dock did in draenor, with enough BOSS KILLS on mythic, it will change to mythic
