Hey guys! I've been quite active on my 19 twink searching around Azeroth for high ilvl gear and maybe things Blizzard overlooked in the pre-patch that allows us twinks to take advantage of! I thought I'd start composing a list here to help others out. Some of this stuff might be stuff you already know and other stuff might be new! I plan to edit this as needed, so bare with me if things are no longer applicable.
NOTE: With Blizzard continuing to hotfix many OP items it might be a safe bet to start farming socketed gear/gems. This will most likely be the BIS gear for most brackets as they nerf high ilvl items and such. Just something to keep in mind!

EDIT: As of 10/20, HoA now has a level 40 requirement to equip, no longer need to obtain.
2. Azerite Armor
EDIT: With HoA now having a level requirement of 40, azerite traits are disabled, but the gear itself is still usable (as of 10/20). The gear obtained pre-HoA nerf has remained the same iLvl. Newly obtained Azerite pieces scale down to an appropriate iLvl, no longer BIS in many cases. I would imagine that this gear will get nerfed someway, somehow with the rapid hotfixes that blizzard has been implementing.
3. Legion Artifact Weapons
Even though the relics have gotten a nerf, two +5 relics mean iLvl 30 weapons for 19 twinks which is a viable option. Do the quest for the weapons themselves, then go farm some legion dungeons or questlines for relics!
EDIT: Druids ONLY: https://www.wowhead.com/item=135582/corrupted-ghanir-the-mother-tree?bonus=0#created-by-spell No longer applicable, they nerfed it into the ground. Complete the restoration artifact questline.
4. Legion Zone End Rewards
Here's a comment from WoWhead that explains the Legion Zone End Rewards from each zone.
The four leveling Pillars of Creation all reward ilvl 50 gear for the final quest.
Yup, ya gotta go back to Warlords of Draenor and make another garrison. Once you get your Garrison to level two you can get some pretty awesome ilvl 40 gear from missions. It appears that only gloves and a ring are available and that is it! Still might be worth doing since you have to do the WoD intro to get to the rare elites anyway!

6. Warlords of Draenor On-Use/Proc Trinkets
It appears Warlords of Draenor On-Use/Proc values were not scaled down on trinkets. If you want to see it for yourself go get an easy trinket in https://www.wowhead.com/item=108902/armored-elekk-tusk. Might be worth checking into more Rare spawn drops and lootable treasure around Draenor!
7. Apexis Crystal "Assault" Rare Elites! (ilvl 44 Gear!!)
UPDATE (10/21 @ 7:00PM EST): It appears that new items received from these rare mobs now scaled to your toon's level, but gear obtained from these rare mobs prior have retained their item level and appear to be grandfathered for now!
EDIT (10/22): It appears that Cloaks, Rings, Necks, and some Shields/Off-Hands still have an Item Level of 44, expect these to get nerfed in the near future, go farm them while you still have the chance!
EDIT (10/23): Blizzard has nerfed all ilvl 44 gear except rings/necks/cloaks even ones obtained prior to them hot fixing new drops. Best of luck on farming your BC dungeon gear and socketed crafting gear!
Here's a Wowhead guide on all Apexis Crystal "Assault" Dailies, we only focus on the rare mobs in each zone as they each have a chance to drop a specific iLvl 44 piece. All you have to do is go to the zones (No need to pick up any quest) and kill rare mobs and hope for good drops. 19 twinks can only do the assault zones in Shadowmoon Valley, Frostfire Ridge, and Gorgrond. Note: It appears that these rare mobs do have a respawn timer that is not instant, so if it's not up it was likely recently killed. The rare mobs do exist!
Note: Here are two additional ways of looking up rare spawns on Wowhead based on items they drop and loot tables. Keep in mind that all rare spawns might not be on these list so do some exploring yourself! Enjoy!

Have someone make a Ghost Iron Dragonling and use a Relic of the past to make it Req lvl 14 and iLvl 19. Then go visit an engineering supplier in a major city (https://www.wowhead.com/npc=5519/billibub-cogspinner in Stormwind for example) and buy some cogwheels with the odd mats that are required to purchase them! These are the only cogwheels that work! The cogwheels made by a Pandarian engineer require an iLvl 38 trinket which our version of the dragonling is not so those do not work. Note: Can only have one kind of a cogwheel in the trinket.
Note: You do have to be an engineer to place the cogwheels into the trinket. But not to equip the trinket.
9. Apexis Shard Unique-Equip Gems (Burning Crusade)
Through this thread, it has been brought to my attention that there are gems that have not been scaled down bought from a unique vendor in Blade's Edge Mountains in Outland! This vendor is https://www.wowhead.com/npc=23244/aether-tech-adept , he is unique in the fact that he only spawns during an event! This event appears to occur on a timer of 2-4 hours (based on my experience) and occurs at the vendor's location in Northern BEM, just Northeast of Besh'ir Landing. This event, which is waves of elite mobs, can prove difficult if not properly geared so be warned. Here is a list of the gems he sells and what stats they give to a level 19 toon:
Note #2: These gems are Unique-Equip, meaning you can only have one of each gem equipped on a character. So don't buy 3 of one gem thinking you'll be able to socket all of them into a chest piece!
For future reference, there is another vendor that spawns after the one mentioned above, https://www.wowhead.com/npc=23245/aether-tech-master. He sells gear as well as Meta gems that have 5% Stun Resistance!
10. https://www.wowhead.com/item=116154/barov-lumberjack-caller?bonus=0 Lumber Mill Trinket Questline!
Thank you for bringing this one to my attention! Here's what ya gotta do to get this trinket. Note: Can do this at Lumber Mill Level 1.
1. Build and complete a Lumber Mill.
2. Do the first couple of quests that go along with it.
3. Then go out into Draenor and find https://www.wowhead.com/object=235094/fallen-tree , I found mine in Shadowmoon Valley. (It appears the spawn rate at each location is not 100% - It would be my guess only one fallen tree spawns per zone and it varies character to character).
4. This will give you a new garrison follower!
5. Go back to your Garrison and get https://www.wowhead.com/quest=36448/reduction-in-force from your new NPC, then finish the quest and you now have a new trinket!
That is all I have for now and is what I'm currently doing on my 19 twink! Let me know if there are things that could be added to this list or something I may be wrong about! Happy hunting all!
