Yes, only lootable off level 21 creatures inside SFK. There are 5 possible level 21 creatures inside SFK, 4 on a normal run.

I believe that shadowfang is actually the BiS weapon for Prot warriors. As rend is affected by weapon damage and shadowfang having the highest dps of any one hander you will have the best possible rend damage. Spreading that rend with thunderclap has lead to big numbers for me recently.
