Since EPlC has a post on formal games and premades I thought I would explain a bit more.
For Arena the complication is that to initiate a game the 2 leaders must target each other so must be on the same realm. So if you had 3 people on Server A and a friend on Server B, you could create a 2v2 match with AA verse AB.
If you wanted to have a pure Server A team verse a pure Server B team you would need to have additional people join and then drop out.
For example to get a AA verse BB match you would start with AAA verse ABB. The 2 A leaders would start a 3v3 game and then drop out leaving the 2v2 pure match.
