How does scaled items work?

Hi, i have recently started playing alot on my twink and im having a blast.
Inbetween bgs im trying to farm gear to get a few pieces that would improve my char, but im having a hard time understanding how the scaling of items work. For example today i found trolls bane which i really want the 14/13 version instead of the 12/12 one i got.
Is there any way of improving the odds that i get a scaled version of an item?

The 12/12 version of https://www.wowhead.com/item=13114/trolls-bane-leggings is the un-scaled version of the item (as evidenced by requiring level 25 to equip, and being item level 30; 5 levels difference), most likely looted from a chest, or dropped by one of the few rare mobs that still drop un-scaled gear (few are found far and between).

The scaled https://www.wowhead.com/item=13114/trolls-bane-leggings you are looking for would be a world drop from any rare or normal mob that has that item in its loot table. However, I am not sure what the exact level requirement would be, as I am not familiar with 29s item scaling. Perhaps someone more knowledgeable than me can chime in, i.e.: @necroaqua @Goesid.
