Try this, if it does not work, then list which ability you want to use and I can modify the macro for it. I used shadowmeld to test it.

#showtooltip lifeblood (or list whichever ability you want to show the cd for)
/cast lifeblood
/cast shadowmeld;
/startattack [mod:ctrl]
