So, I just came back to level one twinking(magelo from 2010 lol) and i'm still rocking sacred charge and thrash blade both wide +15 agi but as i see most twinks atm using thrash blade with lifesteal/fiery and furbolg medicine pouch. Any explanation?
They're exactly the same as they used to be basically, but IMO lifestealing is more useful than Fiery weapon. Sure you do 10 less damage per proc, but it also heals you for 30 or so. 15 agility is good vs. melee since you also benefit from the dodge, while vs. casters where the dodge you get from it doesn't help, I think Lifestealing is better.
You should ALWAYS use the furbolg pouch and then switch to an off-hand weapon when you've lost 100hp, and then switch back before you heal.

You don't have to use any weapon chain vs casters, since they most likely won't dodge a lot, and your procs will be better. BiS 1v1 is Lifestealing on the BoA mace in MH and Pouch/15agi Btb in OH, atleast that's how I like to duel. I do use the pyrium weapon chain vs. melee and hunters though.Updated edit:

BiS for 1v1 on lvl1s is 15ag MH, living steel weapon chain OH.
For PvE or higher level pvp use pyrium weapon chain / living steel weapon chain.
I know it hurts to lose the procs, but the hit and expertise is too pro.

Replace itemhere with weapon name.Living Steel destroys any dodge chance your opponent has even up to level 14 against level 90s let alone less. Casters can also now be dodged making 15 agi bis overall imo. If you are versing a higher level like a level 30 for pve/pvp, I would suggest using a pyrium chain.
Edit: However to answer OP's original question: BiS would be pretty much exactly as Boud suggested, except the enchants - but I'm sure you will have a better idea on those by reading the comments now. Don't forget to swap your furbolg out after you lose 100hp, and equip again to bandage. I'm sure there's a macro around somewhere, if you can't get it to work manually by keybinds/gear setting.

However, those enchants are still more than useful using the aforementioned /equipslot macro, I would be taking a more situational approach. Weapon chains are far superior when dueling things like rogues and higher levels and against melee in general tbh. You can dodge/parry/miss their attacks and they cannot do the same to you, how is that not a major advantage? Fiery/Unholy/Crusader/LS should still be active enchants in your inventory somewhere, but chains with switch outs are simply the most ideal for the majority of 1v1 situations, imo.
