To get defending champion you need to go to the gurubashi arena and loot the chest there when it spawns. BUT you have to loot the Chest using a toon who has gotten the AGM before. If you don't have an agm... You will have to loot it from the chest first before defending champion shows up in the chest
As Leek states here... Just to clarify, if you don't have AGM, you must. Collect TWELVE Arena Masters and turn them in to collect your Arena Grand Master and get the Achievement. You then will be eligible to loot the chest for the heirloom Defending Champion.
In conclusion, you need to loot the chest a total of THIRTEEN times at least on one toon to get the heirloom.

But the caveat is that you have to have gotten the achievement on that toon, not another toon. Example being, someone cannot get it on a level 100 if only their lowbies have AGMs. You have to looted the chest 12+ times and completed the AGM quest. The next time you loot the chest after completing the quest, you will receive your heirloom. It is an cut and dry as it can be.
