To get those blue rare items in chests, have to be running the map all the time? There is only 1 chest each map?

What is the best way to handle them?

I saw the guides, but do not quite understand, do not know if I'm doing it right.
 
I always and only do Stonetalon Mountains for my farm route.

The best way to handle it is choose a route you like, and stick to it every day (or every three depending on the chest) for the foreseeable future.

When you first start, most will be down for the times you run, because they aren't set to that schedule, and random people will run by and open them. So don't get concerned if you are running the route, and you only get 1 chest. Most of the time, even if you are on a schedule, one run will only mean 2-5, thats just the way it goes.
 
Only one chest per area. But it's hard to figure out exactly which chests belong to one area's group. Like, the 3 at Durnholde Keep are in a group (obviously), but the one at Neader's Steed is also in that group. All the chests at Strathmore (I think that's the name - the dilapidated town North of Durnholde) are all grouped, but I think some of the chests in the ogre area are in that group too.

So if you find one chest in an area, you don't have to bother checking the other spots.
 
Unfortunately Blizzard has never been very transparent regarding loot and other things. Obviously if you find one chest in Durnholde Keep, for example, you can assume that the other 2 spawn locations also within Durnholde don't have a chest at them. This doesn't change how many chests are in the whole zone though, as far as I know every single area in a zone could have a chest.
The most I have found in one zone is 6 at one time, and 13 in a whole sweep of all 5 zones mentioned in my guide.

When I was going ham on Stonetalon, I put my hearth at Krom'gar Fortress. It's centrally located to some of the nodes that are overlooked by questers and easy to get to. Thunder Bluff is also a good option. I like to hearth out of some of the more isolated nodes and just fly to the next area.

In addition, it's my belief that it's necessary to get to all the chests in the zone for them to respawn fast. I I think that's why I found a ton of chests even when we've been crz'ed with huge realms. The heavy traffic actually helps; I think. And they take care of that pesky kobold cave, which I hate.

Also, don't be afraid to take rez sickness in certain areas because it's faster than running and you can avoid almost all mobs if you're careful. I don't worry about getting ganked as horde because ally doesn't ever go there, and if it's a 90 it doesn't matter anyway. As [MENTION=8158]Shft[/MENTION] has mentioned, bubbles are a huge assett, as they prevent you from getting dazed. Also having a good hit set is a huge asset so that your CC hits.
