Do not underestimate aff! Since *NO ONE* plays aff, UA is something that most enemy healers aren't looking for, and I can't count the number of times healers will blindy cleanse and get that nice fat silence (and decent damage). My UA was the deciding factor in winning at least a couple games yesterday. And as far as arenas, I would argue that aff lock is mandatory in any 5s comp.
But destro is best now overall, you can pretty much gear how you want, as long as you just want to do damage (fear will still miss a lot), and the burst is really quite nice.
