Your FCing is good, but overall team play/support is lacking at times imo. As a resto druid you're still a healer, even when you're carrying the flag and geared for survivability. I'd recommend investing in mouseover macros and/or a healing addon, they make support healing pretty trivial. You can toss out quick hots to a couple people with minimal effort and continue focusing on doing your FCing thing.
Outside of the obvious examples (teammates dying at 1:03, 8:30), one instance that stuck out to me was running down tunnel with Potatoez at the beginning of the vid. He wasn't at serious risk, but they switched damage to him and got him to around half health. Shift out of the conc. shot there with a quick rejuv on him and keep moving, there wasn't any imminent danger requiring bear form. Sure, it worked out fine there, but small things like that making use of free GCDs can be the difference in a tighter battle (for example, if that hunter moonwalking down tunnel wasn't atrocious and those 3 rogues popped up).
Anyways, I don't play my druids too much and I'm a pretty derp FC, so that's about all the advice I have. Movement/mobility are good, especially with the juking. Looks like you have the jumps down well enough that you don't need to practice anymore
