Even doing the very first quest in Org or Stormwind will deactivate your necklace. Playing skirms with a 50 lock and a 50 druid (myself). I did the very first quest and my neck is gimped, while my buddy just logged in and still has it working in games.
Both level 50s, both just logged in, but I did the very first turn in and lock did not. My neck still works outside of Arenas btw. But not in them. Super weird. EDIT: oh wait this is just a visual bug lmao

Yes, they're a thing. People have been gearing their twinks since the second half of BFA in preparation for expansion capped accounts. Many people have "started to twink" at 50.
