I can confirm, level to 24 did BFD a couple of times and still looting requires lvl 16 armor

Imo the best way to farm lvl 19 BoEs are SFK chests. Solid + Iron Bound have given me these so far;

I have heard it is possible to get Sphere from Ornate Bronze Lockboxes but I don't have proof.

Mystic's wrap and sphere no longer scale to BiS. Unscaled they are still BiS. Its hard to tell from wowhead if they drop from chests as they are so rare. My guess is that they can be obtained from either chests or lockboxes (see the thread on scaling BVB to 19) - but I don't know that anyone has proof as of yet.

Yeah exactly, im wondering if kellers girdles lvl 18 is also GFd? or is it possible to get a 19 version of it ?

Keller's girdle doesn't scale exactly to level as its a world drop. The ilvl 23 (required lvl 18) unscaled version still drops from chests. It also scales to ilvl 23 (require lvl 18) with the normal scaling scheme for blue world drop armor, which is it drops at ilvl x3 and x8 with a required level of ilvl-5.
Greens are a bit weirder and don't have as well-defined of a scheme.

I can confirm that the level of NPCs does matter when it comes to greens. Farmed level 24 - 25 mobs in southern barrens on my 19 twink and almost every green item had req level 16 with ilvl of 19. Also farmed in Duskwood where the mobs are 18 - 19 on the same toon and the green drops scaled to req 13 and with an ilvl of 16. So indeed the level range of mobs has an impact on what loot you'll get.

I had this theory that if you could find level 27-28 NPCs to farm it may drop level 19 greens, but difficult to prove.

There is already enough proof now how the scaling system works as exlained in this thread.
a short summary (anyone feel free to correct me )
world drop boe's will never scale to 19 doesnt matter what lvl the npc or yourself has.
They will scale either 16 -20 etc.. for gear or 10 -15 etc.. for weps.
Specific zone loot will scale to your level when looted from an npc OR Chest/lockbox.(this includes Specific loot from RARE npc)
Loot from chest or lockboxes is unscaled except when zone specific drop. (#Pray4BVB)

You also have a chance for certain rares and epics to scale to 17-27 etc with +8 ilvl
