Implosion+Trinket macro, along with the postmaster set and book of the dead would be an interesting 49 to play.

Can confirm opness, just got this drop on my rogue I'm working on getting to 60. Currently lvl 50 used it on my friend in a duel and it hit him for 8k. our max hp is around 9k so yeah pretty powerful even at 50...
