They are from the boxes, just very low chance. Maybe 0.2% or something.

Is the mobile app showing these correctly? I swear I remember buying relics when I started twinking only to find that they were incorrectly displayed in the mobile app.

Nope, the mobile app probably shows all the relics as being 810 with 101 req, even if they're level scaled or 110 warforged.
